Purpose of the Position

To ensure the effective day-to-day coordination of the multidisciplinary team and the organized delivery of socio-rehabilitation services, contributing to the rehabilitation process of beneficiaries.

Key Responsibilities

• Coordinate the daily work of the team and service delivery

• Support the organization of case management processes

• Monitor the implementation of individual socio-rehabilitation plans

• Ensure continuity of services and beneficiary engagement

• Organize referrals and cooperation with community-based services

• Oversee documentation and reporting processes

Requirements

• Higher education in Social Work or a related field

• At least 5 years of professional experience

• At least 2 years of experience in team coordination

• Experience working with vulnerable groups is an asset

• Strong organizational and communication skills
