The Armenian Association of Social Workers (AASW), within the framework of the project “RISE – Armenia: Rehabilitation, Social Inclusion and Empowerment of Displaced and Vulnerable Population,” announces a call for applications for the formation of the professional team of the Ashtarak Multifunctional Social Care Service.

Position: Team Lead (Ashtarak Social Care Multifunctional Service)
Project Title: “RISE – Armenia: Rehabilitation, Social Inclusion and Empowerment of Displaced and Vulnerable Population.”
Project Objective: To improve the social protection system in Armenia for vulnerable populations, including those displaced from Nagorno-Karabakh, through strengthening the capacities of social workers and ensuring access to new social services, including patronage, social care, and socio-rehabilitation services.
Project Duration: November 2024 – November 2027

- Scope and Purpose
To ensure the effective day-to-day coordination and operational management of the multidisciplinary team delivering both day-care and mobile (outreach/home-based) social care services within the Ashtarak Multifunctional Social Care Service.
- Duties and Responsibilities of the Supervisor
2.1 Operational and Team Coordination
- Coordinate the daily activities of the multidisciplinary team, including social workers, psychologist, nurse, family mentor, and social skills educator.
- Develop and monitor work schedules, including home visits and group sessions.
- Monitor timely implementation of planned activities and interventions.
- Address operational challenges and escalate complex issues to the Service Supervisor when necessary.
- Ensure smooth internal communication within the team.
2.2 Case Coordination and Service Implementation
- Support the organization and follow-up of individual case management processes.
- Monitor the implementation of Individual Intervention Plans to ensure adherence to approved objectives and timelines.
- Participate in case discussions and case conferences.
- Ensure coordination between day-care and mobile services for continuity of care.
- Facilitate referrals and follow up on service provision outcomes.
2.3 Coordination of Day-Care and Mobile Services
- Coordinate services delivered within the day-care center.
- Ensure proper planning and implementation of mobile (outreach) services.
- Monitor attendance and participation of beneficiaries in day-care programs.
- Ensure that service delivery complies with established standards and procedures.
- Support the Supervisor in ensuring service accessibility and continuity.
2.4 Community and Stakeholder Coordination
- Facilitate communication with Local Self-Government Bodies, the Territorial Center of Social Services, and health, education, employment, and other social institutions operating in the community.
- Support organization of coordination meetings with health, education, employment, and other social service providers.
- Maintain updated information on available community resources and referral pathways.
- Documentation, Reporting, and Data Management
- Ensure accurate and timely documentation of service delivery activities.
- Monitor proper case documentation and filing by team members.
- Prepare periodic operational reports for submission to the Service Supervisor.
- Contribute to data collection processes and service performance tracking.
- Ensure compliance with data protection and confidentiality standards.
2.6 Other Responsibilities
- Support the Service Supervisor in internal monitoring processes.
- Participate in donor visits, coordination meetings, and evaluation processes when required.
- Perform other duties as assigned within the scope of the position.
3. Minimum Qualification Requirements
- Higher education in Social Work (Master’s degree is an advantage).
- At least 5 years of professional experience in social services or community-based programs.
- At least 2 years of experience in team coordination or project implementation.
- Experience working with vulnerable populations, including displaced persons, is an advantage.
- Strong organizational, coordination, and communication skills.
- Computer literacy (MS Office and basic data management tools).
